Service agreement acknowledgment

The final lineup, fee, performance schedule, production scope, cancellation terms and deposit amount will appear in the countersigned agreement.
Client will provide safe access, the agreed performance area, two suitable power circuits and any venue permissions.
Deposit may be paid by Zelle or mailed check using private instructions supplied after acceptance. Never send funds to details not confirmed directly by Doc Hereford.
No date is reserved by this form alone. The agreement and deposit must both be completed.
